Engagement Letter for Individual Tax Return
For each year you hire us to prepare your individual tax return, we are required to have an engagement letter on file.  Feel free to print and sign this form and forward to our office.

Consent to Use of Tax Return Information
In our effort to fully serve our clients, from time to time, we provide helpful information such as firm newsletters, published articles, press releases, and announcements related to firm personnel.  To determine what items may be of interest to you, we may need to use your tax return information.  If you agree to the use of your tax return information for this purpose, you’ll need to print, sign, and date this consent form.  This is a required form.

If you would like us to disclose your information to a third party (usually a bank or financial institution), you will need to complete this form detailing which tax return information information we may disclose, to which institution we may disclose it, and for what purpose.  Please print, complete and sign this form and forward to our office.  This is not a required form.
